Blue Care Network plans emphasize prevention of disease and illness as

a primary care physician from the more than 3,700 health care providers
in the network. The PCP provides, manages and authorizes most of your health care services. BCN offers comprehensive coverage with no deductible and affordable copays. Health education, disease management, care management and pharmacy programs help you get healthy and stay healthy.

Healthy Blue Living℠
You are rewarded for adopting and maintaining a healthier lifestyle with Healthy Blue Living. You automatically receive lower copays and deductibles for the first 90 days of coverage. To remain eligible for the lower cost-sharing, you and your spouse need to complete a health risk appraisal, meet with your primary care physician to complete a wellness commitment form and adopt a healthy lifestyle as recommended by your primary care physician.
